    National Council on Aging (NCOA)

  • Better Choices, Better Health - Diabetes is a six-week online workshop teaches individuals with diabetes self-management skills and techniques to more effectively manage their diabetes, overcome challenges, and improve their health. The workshop provides tips and tools to create and track weekly plans to improve health; monitor blood sugar, food, medication, and exercise; and an outlet to discuss problems and solutions with other participants.

    National Diabetes Education Program (NDEP)

  • ChooseMyPlate.gov from the USDA illustrates the five food groups that are the building blocks for a healthy diet and provides tools for diet/calorie tracking and information on weight management, physical activity, and healthy eating. Some materials available in Spanish.

    U.S. Department of Agriculture (USDA)

  • This program brings face-to-face, small-group sessions to life through conversation maps.  With the help of a trained facilitator, participants focus on the health information most relevant to them and relate it to their own experiences.  At the end of each session, participants create a workable plan for using what they learn to drive meaningful behavior change in their lives.  Training for facilitators is available.
